The National Forum on Hospitals, Health Systems and Population Health app is used in conjunction with the medical community to access Hospitals, Health Systems and Population Health content. It is updated with the latest information on related summits, houses archived content from previous summits, and can be used to interact with current summits in real-time for attendees and online viewers alike. The app features current information about the most recent or upcoming summit (speakers, agenda, sponsors, etc.).

About The National Forum on Hospitals, Health Systems and Population Health

Increasingly, health care providers and other stakeholders are looking to embrace population health strategies to create a community-wide "culture of health".

There is broad recognition that the poor health status of many Americans requires new strategies to address upstream drivers of health.

Effectively managing populations of patients requires new partnerships among providers and payers and the traditional public health community.

Participants will learn how to work across sectors to address these drivers, and build new bridges among health care, public health, and community development.

Strategies pursued include new uses of data, analytics and information technology; augmenting a non-traditional health care workforce; and employing new delivery and payment models.

Participants will learn how leading systems are drawing on these new tools and arrangements to improve the health not just of their own patients, but of overall communities.

Nonprofit hospitals and health systems face new obligations under the Affordable Care Act to undertake community health needs assessments and implement community health improvement plans.

Participants will learn how the most innovative systems are embracing and executing this new responsibility.
